In 1917 Marcel Duchamp didn't quite get his "fountain" scuplture/readymade into an exhibition despite sitting in the selection panel. There's a full account here of the incident.

These toilets seem to be in the institution, but not of it, much like Duchamp's. Though, of course his didn't make the institution until he had copies made, but that's another story.
